Nursing care for patients with chronic obstructive pulmonary disease with ineffective airway clearance: case report

Gradual narrowing of the airway caused by Chronic Obstructive Pulmonary Disease (COPD), gas exchange is
impaired, and breathing becomes difficult. COPD is a long-term condition that cannot be cured. Preventing
symptoms from worsening and making the patient more comfortable should be the top priority of healthcare
providers when managing this disease. COPD can be emphysema (a lung disease that causes damage to the
alveolus), chronic bronchitis and small airways disease which are pathologically distinct but can occur
simultaneously. According to the WHO, chronic obstructive pulmonary disease (COPD) ranks as the third worst
killer in the world. Approximately 3.7% of Indonesia's population, or 9.2 million people, suffer from chronic
obstructive pulmonary disease. This study aims to determine changes in the condition of palliative patients with
COPD after nursing interventions for patients and families. This research method uses a case study method with
observation, interviews, access to medical records, using a monitor to determine the patient's vital signs, chest
physiotherapy health education is carried out in the patient's room, carried out to the patient's family with leaflet
media accompanied by demonstration methods and direct practice to patients. The conclusion of this study is that
chest physiotherapy intervention, collaborated with pharmacological therapy mucolytic n.ace and combivent
nebulizer, has proven effective in reducing the problem of ineffective airway clearance in palliative patients with
copd. Suggestions for future researchers can use other health education methods with other suitable topics to
reduce the problem of ineffective airway clearance in COPD patients with palliative conditions who experience
decreased awareness.
